Be There San Diego is an initiative of the San Diego Wellness Collaborative, a nonprofit organization with the mission of building community-clinical linkages and collaboration for optimal health outcomes and health equity.
The San Diego Wellness Collaborative serves as the backbone organizations for three primary initiatives: Be There San Diego, Neighborhood Networks, and the San Diego Accountable Community for Health.
San Diego Accountable Community for Health is a multi-sector initiative working to create community health, wellness, and equity.
Neighborhood Networks is a social enterprise that addresses the health-related social needs of community members using a coordinated approach that links health plans with community-based organizations and trusted community health workers.
